We have a new medical director /supervising physician that I am trying to get attached to our office with CMS. I have filled out several different forms and talked to CMS a couple of times just to be told yet a different form. Since he is not seeing patients does anyone know which form would be correct to just attach him to our practice as our new medical director since he is not seeing patients. Thanks much. 7/22/2020 @ 3:32:00 PM

CMS Form 29 is used to update the medical director. This form is submitted to your state agency who will forward it to CMS.

That form 29 states it is for the medicare program to complete and
return to the State and also is used when the state agency surveys an RHC. That is confusing. We need to add this provider to our clinic and mark it that he is a medical director only so he can sign papers and we can bill under his name as necessary. On 7/22/2020 3:32 PM, NARHC News wrote: > |
