You can only charge for no shows if the policy applies to all financial classes of patients consistently. The problem is that most Medicaid programs do not allow you to charge a missed appointment fee so the policy, if you have one, cannot be applied to all financial classes.
Charging for missed appointments is usually not a very productive or patient-friendly policy. It is punitive, will make patients mad, and make them leave your practice anyway. It is far more positive and productive to find out why the patients are missing appointments and try to find a solution that is win-win. Many of our clinics do QAPI projects on missed appointments and try to help solve the problem. I don't recommend charging for missed appointments. It usually backfires. Attached is our No-Show policy. The appointment failure is communicated to our back office staff, who attempt to contact the patient and find out why they were unable to make their appointment and reschedule if necessary. If they do not respond or provide a valid reason, a no-show letter is sent to them. If they have three unexcused no-shows within a year's period of time, they lose scheduling privileges, and this is communicated to the patient via certified mail. Patients are then required to make five same-day appointments, meaning that they must come to the clinic and wait to be worked into the schedule to be seen, before their scheduling privileges are reinstated.
