We are updating our flu vaccine policy in preparation for the arrival and distribution of the vaccine. We are a provider-based clinic in Iowa with Epic as our electronic medical record.

Does your clinic have patients review the questions (fever today, allergy to eggs, history of Guillain-Barre syndrome, sensitivity reaction) and sign they have reviewed this information prior to administration?

My research has come up short stating this is necessary documentation.

We do have our patients review form and scan into record.. this also allows us to give flu vaccines to our schools and nursing homes that do not have the best internet connections somedays

All flu vaccines given must have the waiver signed and on file prior to
administering the flu vaccine for all patients. If not this becomes a risk
issue if you did not and the patient suffers a reaction to the
administration of the vaccine.
