Provider Patient Encounter Requirements

As a RHC, our Providers are required to have a certain number of patient encounters per year. Can anyone tell me the minimum number of encounters required? Is it per provider or the entire tax ID? Medicare Provider Reimbursement Manual , Cost Reporting Instructions state:

2907.1 Part I - Visits and Productivity: Productivity standards established by CMS are applied as a guideline that reflects the
total combined services of the staff. Apply a level of 4200 visits for each physician (1.0 FTE) and a level of
2100 visits for each nonphysician practitioner (1.0 FTE).

These numbers can be combined to meet the minimums. This is the only thing I can recall that specifically states a number of visits.
