CPT 99473 and 99474

Can an RHC bill for CPT codes 99473 - (self-measured blood pressure using a device validated for clinical accuracy; patient education/training and device calibration) or 99474 (separate self-measurements for two readings one minute apart, twice daily over a 30 day period; collection of data reported by the patient to the physician, with report of average systolic and diastolic pressures and subsequent communication of a treatment plan to the patient)     if seen by a provider?  And would it be considered "incident to" if only seen by a nurse?

These services would not be an RHC encounter. Also, RHCs are excluded from being able to separately bill for any remote patient monitoring This is considered incident-to other services or to care management. Unfortunately, we have many vendors who are not aware of the RHC requirements.
