Need some clarification on billing for the COVID test in our RHC  .We bill CPT 99213 with revenue code 521 for the visit.  Should we use revenue code 300 or 521 to bill for the lab test which is CPT 87426?  Is there a modifier required if the 2 are billed on one claim together?

If you are an independent clinic then you would bill the lab under your Part B # to Medicare. If you are a Provider Based clinic then the lab is billed under the hospital numbers, just like the patient walked into the hospital to have the service rendered.
