Unsuccessful Procedure Performed in the RHC

When the provider performs a procedure but it is unsuccessful, how should that visit be billed in the RHC? Are others reporting the procedure or the office visit E/M? If reporting the procedure, are you adding modifier 52 or another modifier? Examples: The provider attempts to reduce a shoulder dislocation but it is not successful. The provider attempts to remove a foreign body and makes an incision; however, is not able to find the foreign body. The provider inserts a pessary but it is not the right size. Thank you!
