Provider Home Visits and Medical Necessity

I'm looking for regulatory guidance for provider visits in the home. I am familiar with the RHC regulatory guidance for VNS, however, looking for specific detail on established guidelines for the provider doing home visits. Medical Necessity Requirements, etc. Appreciate any information shared!

The home encounters would have to meet the same definition for an encounter found in the Policy Benefit Manual, Chapter 13. It must be a service requiring a face to face visit. CMS removed the requirement that the patient be homebound a while back.
