Can someone please tell me the regulation regarding PPD vials? Is this considered a MDV and requires a 28-day expiration date? We have a small RHC, and will not use the PPD vial within 28 days--possibly only getting one dose out of it.

Yes. We consider it a MDV and dispose of it at the 28 day marker.
