If you bill the G0511 or G0512, is the patients coinsurance from Medicare 20% of billed charges or 20% of the CCM rate for G0511 or G0512?

It should be 20% of the lower of the charge or the reimbursement. This changed. Previously, it was 20% of the charge if it was higher than the reimbursement.
