Hello All, I am getting to ready to launch our new Chronic Care Management in our RHC. I am currently in the hiring process of the position, but have done much of the ground work in terms of research of the program (billing, coding, regulations, EHR requirements, possible workflow processes). I am reaching out to see if anyone has implemented a Chronic Care Program in their RHC and if they could offer some insight and resources if at all possible. I am also reaching out to see if there is any good educational, convention or conference tools/resources, or certification for chronic care management. Thank you so much! We've are a 7 provider RHC in NE Florida. We started CCM In 2017 with 2 care managers. We now have 4 care managers and would like to hire more.
