We are a Provider Base RHC.  If Providers have a face to face visit & the Nurse gives a Shingrix Vaccine to the patient. When billing Medicare what REV Code do you use?

Shingrix is not payable by Medicare Part B. All vaccines (except Flu, Pneumonia, and Hep B) are part D. If the Part D vaccine happens withOUT an office then you could bill a Bill Type 710 with condition 21 and receive Patient Responsibility denial to bill the patient 100% of the charge. If the vaccine happens with a visit, then you would have to write off the vaccine, as you cannot have part of the visit as non-covered and part covered. The best is to send the patient to the pharmacy to receive the vaccine or sign up with TransAct RX to be able to bill Medicare Part D vaccines and receive payment.

The Shingrix immunization is a Medicare Part D benefit and is not separately payable to the RHC. Immunizations are a Part D (pharmacy) benefit with the exception of Hep B and tetanus in some cases which are part of the AIR. However, no immunizations other that flu and pneumococcal are separately reimbursed to the RHC. These are reimbursed as part of the a cost report calculation.
