We are working on updating clinic documentation requirements for our providers, and are wondering where other facilities document the information for timeliness of chart completion. Is it part of clinic bylaws, do you have a policy, is it part of the provider contract? Also, how long do you give providers to complete clinic notes before they are considered out of compliance? Thank you in advance for feedback.

Yes we have a policy and medical staff helps enforce. Also provider incentives are tied to it.
